2027 Medicare Changes in San Antonio: What You Need to Know

If you have Medicare in San Antonio or anywhere in Bexar County, 2027 may bring changes to your health coverage, prescription drug coverage, costs, provider network, or additional benefits.

That does not automatically mean you need to change plans.

It does mean you should take a few minutes to review your coverage before the new year.

At Medicare Pro, we believe Medicare decisions should begin with education—not pressure. Here are some of the most important things Medicare beneficiaries should know heading into 2027.

1. Medicare Open Enrollment Runs October 15 Through December 7

Medicare Open Enrollment takes place every year from October 15 through December 7.

During this period, people with Medicare may be able to:

  • Change from one Medicare Advantage plan to another Medicare Advantage plan.

  • Change from Original Medicare to a Medicare Advantage plan.

  • Leave Medicare Advantage and return to Original Medicare.

  • Join, change, or drop Medicare prescription drug coverage when eligible.

Changes made during this enrollment period generally become effective January 1, 2027. Medicare confirms that the annual Open Enrollment Period runs October 15–December 7.

Remember: simply because Open Enrollment is happening does not mean you need to change your coverage. If your existing coverage continues to fit your healthcare and financial needs, staying with your current plan may be appropriate.

2. Review Your Annual Notice of Change

If you currently have a Medicare Advantage or Medicare prescription drug plan, one of the most important documents you can review is your Annual Notice of Change, commonly called the ANOC.

Plans send this notice each fall to explain changes that will take effect the following January.

Your ANOC may show changes involving:

  • Premiums

  • Deductibles

  • Copays or coinsurance

  • Prescription drug coverage

  • Pharmacy coverage

  • Additional benefits

  • Other plan rules or costs

Medicare recommends reviewing this document to determine whether your current plan will continue meeting your needs in the coming year.

Don't automatically assume that the plan you had in 2026 will work exactly the same way in 2027.

3. Prescription Drug Costs Are Changing in 2027

Prescription drug coverage deserves special attention this year.

For 2027, the maximum deductible under the defined standard Part D benefit is $700, up from $615 in 2026.

The 2027 annual out-of-pocket threshold for covered Part D drugs is $2,400, compared with $2,100 in 2026. Once the applicable out-of-pocket threshold is reached, beneficiaries pay no additional cost sharing for covered Part D drugs during the catastrophic phase.

These are federal Part D benefit parameters. Your individual drug plan may have a lower deductible or a different cost-sharing structure, so always review the details of the specific plan you are considering.

This is one reason we encourage beneficiaries to review more than the monthly premium when comparing prescription coverage.

4. Check Your Prescription List Every Year

A plan may have worked very well for your prescriptions last year and still deserve another look for 2027.

When reviewing drug coverage, consider:

  • Are all of your current medications covered?

  • What tier is each medication on?

  • Does the plan have a deductible?

  • Are there prior authorization requirements?

  • Is your preferred pharmacy still participating?

  • Would another participating pharmacy have different costs?

  • Are there quantity limits or step-therapy requirements?

Medicare's official Plan Compare tool allows beneficiaries to enter their ZIP code, prescriptions, and pharmacy information when comparing available options.

The lowest-premium plan is not necessarily the lowest-cost plan once prescriptions are taken into consideration.

5. Make Sure Your Doctors Are Still in Network

For many San Antonio Medicare beneficiaries, keeping their doctors is one of their highest priorities.

Before changing Medicare Advantage coverage, check important healthcare providers such as your:

  • Primary care physician

  • Cardiologist

  • Endocrinologist

  • Oncologist

  • Orthopedic specialist

  • Hospital

  • Medical group

  • Other frequently used specialists

Medicare Advantage plans may use provider networks, and network requirements vary by plan. Medicare notes that Medicare Advantage members may need to use providers within the plan's network and service area for non-emergency care.

We recommend verifying important providers with the plan and, when appropriate, with the provider's office before making a coverage decision.

6. Don't Choose a Plan Based on One Benefit

Dental, vision, hearing, transportation, over-the-counter allowances and other additional benefits can be valuable.

But they should not be the only reason you select coverage.

A Medicare plan should be evaluated as a whole.

Ask:

Does it cover my doctors?

How does it cover my prescriptions?

What could I pay when I actually use healthcare?

What are the plan's network rules?

What are the copays, deductibles and maximum out-of-pocket costs?

Do the additional benefits actually fit my needs?

A large additional benefit may not make up for losing an important physician or having significantly higher medical or prescription costs.

7. Medicare Plans Can Be Local

Medicare Advantage and Part D plan availability can vary by service area.

That is why someone living in San Antonio may have different options from a friend or family member who lives in another part of Texas—or sometimes even another nearby county.

Your ZIP code, county, eligibility, doctors, medications and healthcare needs all matter.

There is rarely a meaningful answer to:

“What is the best Medicare plan?”

A better question is:

“Which available coverage option fits my individual situation?”
